Background: Alzheimer's disease is an important challenge in worldwide. Various technologies, such as electronic personal health records based on mobile phones (ePHR-app), involved patients in care at a low cost, and as a result, prevented the progression of the disease and managed their health during the Covid-19 pandemic. Objective: This study was conducted with the aim of designing and developing an ePHR-app for managing the health of Alzheimer's patients during the Covid-19 era and post. Material and Methods: This developmental-applied study was conducted based on the Design Science Research Methodology. The needs of the patients were extracted by reviewing related articles and conducting semi-structured interviews with physicians (n=7), caregivers (n=13), and two focus group meetings (n=8). The initial version of ePHR-app was designed and developed based on the findings of the previous two phases. The ePHR-app was implemented based on the Android platform. Results: The content requirements (e.g. clinical and medication history), structural features (e.g. technical features and data display methods according to users' needs), and essential capabilities (e.g. reminders and periodic reports) extracted from the interviews and used for the system design. Conclusion: In this study, an ePHR-app was designed and developed for Alzheimer's patients. Early involvement of end-users in all stages of design, development, and implementation improves the acceptance of the ePHR-app. Using the ePHR-app can be useful in improving the health of Alzheimer's patients and providing effective support to physicians and caregivers.
